docs(code-graph): stage-based bulk index commit scenario in verify spec

Rewrite the bulk index transfer scenario: data is staged to a worker-side
session in bounded chunks, the batch commit makes the full session visible
atomically in a single worker transaction, and pre-commit failures leave
previously committed graph state intact.
