feat: add RESPONSE_CACHE_ENABLED env to disable response cache

Cherry-picked from PR #142 by @suhaihui-git — adds isCacheEnabled() guard
to cacheGet/cacheSet and exposes enabled field in cacheStats(). Defaults
to enabled (backward-compatible).
