Add support for standard SSH port forwarding

This commit adds support in SSHClient and SSHServer for easily
requesting standard SSH port forwarding. It also adds some new examples
showing how to use these capabilities and makes the Sphinx doc files
part of the repo.

There's also some minor cleanup of other documentation in here, and a
debug print statement accidentally left in v0.1.0 has been removed.
