Release v0.5.1: Windows compatibility and CLAUDE.md refresh (#16)

* Add Windows compatibility and refresh CLAUDE.md with auto-memory

Add python3/python fallback in hook command for Windows where python3
is not available. Refresh CLAUDE.md to auto-memory format, removing
ephemeral data and adding git-insights and best-practices sections.

* Bump version to v0.5.1
