fix: exclude same-millisecond replayed usage from scan budgets (#631)

Exclude replayed parent usage from child scan budgets when parent and child IDs share a millisecond. Apply the pinned Codex producer ordering consistently in the live tracker and Python collector, with focused attribution and installed-cache upgrade coverage.

Advance the paired plugin version markers to 0.1.95 so existing cached installations receive the corrected collector.

Validation: current-head Node and container CI passed. The focused 0.1.94 cache-upgrade test also passed locally, including 26 assertions.
