Single Execution (#545)

Only allow a single execution of a workflow across the cluster (database
determines global, local map also).
Fix issue with repeated app calls bumping the recovery count up.
Fix recovery of temp workflows.
Handle system DB connectivity hiccups when starting workflows and
recording step outputs.
Fix naming conflicts for named workflow parameters (names like `status`
used to throw errors)
