Improve MAX_RECOVERY_ATTEMPTS Semantics (#580)

- Fix an issue where a workflow throwing MAX_RECOVERY_ATTEMPTS_EXCEEDED
could disrupt recovery
- Fix an issue where the MAX_RECOVERY_ATTEMPTS_EXCEEDED was not
recognized as terminal by `getResult`.
