test(worker): kill SheetNames fallback mutant; mark true equivalents

Assert non-array SheetNames imports nothing so the empty-array fallback
cannot be replaced with a phantom name. Document two equivalent mutants:
max reassignment on equal values, and toLowerCase undoing any toUpperCase
before prepareAppendImportPlan re-normalizes format.
