provider-test-coverage: make report self-explanatory

Add a plain-language intro and key-terms legend explaining what the
report measures, how to read X/Y (passed/observed), and that 0% Strict
usually means strict-only checkpoints were not run (not broken). Label
the provider table columns and the per-pair gaps section. Put readable
checkpoint labels before raw ids.
