fix(fetch): scope smart_fetch focus context and forward to bulk URLs (#27)

By @robbyczgw-cla. Scopes smart_fetch request context to each invocation via a decorator that resets ContextVars in finally, preventing focus from leaking between sequential calls. Forwards focus through bulk URL fetches. Fixes #24.
