feat: Streamline README and fix AGENTS.md frontmatter docs

Simplified README by removing redundant content, JSON examples, and the
architecture table. Fixed AGENTS.md description to correctly note that
plan-files.ts uses the SDK frontmatter parser instead of its own
regex-based parser.
