feat: Codex runtime-state intelligence + live quality refresh (v5.8.0)

Codex parity:
- Read-only codex_state reader: subagent token cost + leak detection,
  memory overhead, and goal-budget utilization from Codex SQLite state
- Modern session parsing: collab subagent events, compaction compression,
  rate limits, per-turn effort, and tool durations
- codex-state command + dashboard surfacing; SubagentStart/Stop sprawl hooks
- Doctor and status-line updates for the current Codex surface

Quality bar:
- Context quality now refreshes during long autonomous runs (throttled
  PostToolUse), so the score reflects live session state
- Per-session fill attribution keeps concurrent sessions independent
- Status line marks a non-live score
