phase 3 review fixes

Two issues caught:

1. pd-array-confluence.md: Formal Criteria said 'high-confluence has
   4+ factors' but Formula/Math said 'high = 5-6 factors.' Self-
   contradiction. Unified: medium = 3-4, high = 5+, low = 1-2.

2. ote-overview.md Example 1: bad math. Said '-1.5 SD ~145 pips' and
   'R:R = 9.7R' but the actual distance from entry 1.0830 to target
   1.1050 is 220 pips, R:R = 14.7R. The companion file ote-705.md
   already had the correct number — fixed ote-overview to match.
