fix(wxo): log create failures with traceback and map agent 409 to conflict

- Log deployment create failures at exception level before rollback so root
  cause is visible in server logs.
- Translate WXO agent client 409 responses into DeploymentConflictError with
  provider detail for consistent API responses.
