Disable MSAL for now (#235048)

Because of late breaking issues:

* https://github.com/microsoft/vscode/issues/234932
* https://github.com/microsoft/vscode/issues/234954

This also comments out the `_setupRefresh` logic since that is causing the high CPU load. I'd like to see what happens when we don't do this behavior.
