refactor: remove dead code and unused config knobs

Drop the orphaned update.sh module and its test, the unreachable history
case and show_update_notification in mole, get_darwin_major in base.sh,
and the unused MOLE_AGENT_WORKTREE_SCAN_SEC override. None had production
callers.
