Improve layout fragment height calculation to handle incomplete layout cases

Use max() when computing container height from layout fragments to ensure
the height does not shrink if lazy layout hasn't completed to the end of
the document. Also remove an unnecessary debug log and clarify comments
related to FB15131180 workaround.
