refactor: retire index-parallel command

- index-parallel removed; index command uses the same Pipeline::index_incremental path
- save_document_index_metadata removed from IndexPersistence (only caller was index-parallel)
- CLI integration test updated to use index command
- 401 lines removed, 4 added
