fix: use Setup hook instead of SessionStart for env var timing

CLAUDE_ENV_FILE vars from SessionStart hooks apply after the API
client initializes. Setup hooks run earlier, before the session
starts, ensuring ANTHROPIC_BASE_URL is set when the API client
reads it.
